Everything about FTM top surgery

Type of procedure

This gender-affirming surgery is designed to masculinize the chest, achieving a flat and harmonious appearance for trans men or non-binary individuals wishing to align their body with their gender identity.

Surgery

Horizontal mastectomy is performed under general anaesthesia and involves removing breast tissue through a horizontal incision, allowing the pectoral contour to be defined while simultaneously adjusting the position of the nipple-areola complex.

Recovery

Recovery usually lasts between 2 and 4 weeks. During this time, it is recommended to wear a compression garment to aid skin adaptation and reduce swelling, and to attend all follow-up appointments for optimal results.

Benefits of horizontal mastectomy

Alignment with Gender Identity

The result is a flat, masculine chest that helps patients feel more comfortable with themselves and regain body confidence.

Reduction of gender dysphoria

Many patients experience discomfort due to breast tissue. After surgery, this discomfort diminishes, improving overall well-being.

Greater freedom of movement

Say goodbye to binders, back pain, and skin irritation caused by prolonged use of compression garments.

Defined masculine chest contour

The horizontal technique is designed to create a flat, defined chest, following safe medical protocols with high precision.

Less visible scars

The placement of the incision makes scars less noticeable, resulting in a more natural and discreet appearance.

Postoperative care

Postoperative care is essential for a safe and smooth recovery. Here’s how to take care of yourself after surgery.

Wound care

Keep dressings and bandages on as directed by your doctor. Clean the scars with saline solution and pat dry gently without rubbing.

Physical activity

Avoid lifting heavy objects and any activities that strain the chest area for at least 2–4 weeks.

Drains and compression garment

Drains and a post-surgical compression vest will be applied after surgery. Your surgeon will advise wearing them for one month to reduce swelling.

Follow-Up Appointments

Attend all post-operative visits to monitor your recovery and scar healing. You can also raise any questions during these appointments.

Medication

Painkillers and anti-inflammatory medication may be prescribed to relieve discomfort and reduce swelling.

Avoid smoking

Smoking negatively affects healing and increases the risk of infection or complications. It is recommended to avoid smoking during recovery.

What is FTM top surgery?

FTM top surgery is a gender-affirming surgical procedure designed to create a flatter, more masculine-appearing chest. It is most commonly performed for transgender men and non-binary individuals who wish to reduce or remove breast tissue in order to align their physical appearance with their gender identity.

This procedure, focused on breast tissue removal, is often necessary for serious medical conditions, such as breast cancer or high risk of developing it. It is also a key gender-affirming surgery for transgender individuals, creating a flatter, masculine chest and significantly improving emotional well-being and gender congruence. Choosing a trusted medical team ensures support and confidence throughout the process.

Before FTM top surgery, a comprehensive health assessment is conducted, including imaging – mammograms or MRI-, blood tests, and a detailed medical history review.
